Lab Membership Agreement

The undersigned individual, hereby indicates their desire to become a member of ​EVOLVE HUMAN OPTIMIZATION LABS, pursuant to the terms and conditions of this Lab membership agreement.

Health Attestation

The Member attests that they are fully able to participate in an recovery regimen of their design or choosing without undue risk.

Lab Membership Dues

The Member agrees to pay either monthly subscription or day pass access, indicated by selecting an option in the package section of their profile. These dues may be paid by automatic debit/ACH or credit card transaction or in-person via debit or credit.

Member Access

This Lab membership agreement entitles the member to access ​EVOLVE HUMAN OPTIMIZATION LABS facilities so long as all dues are current.

Indemnification

This lab membership serves as a liability release, pursuant to which the Member agrees to indemnify and hold ​EVOLVE HUMAN OPTIMIZATION LABS harmless against any and all claims of loss or damage without limitation.

Safety Guidelines:

The Member hereby agrees to abide by all posted safety guidelines and regulations while using ​EVOLVE HUMAN OPTIMIZATION LABS facilities and equipment. 

Additionally, the Member agrees to dress and conduct themselves in a manner deemed appropriate for a recovery facility.

Prohibited Substances:

Consumption of drugs, alcohol, or tobacco products on EVOLVE HUMAN OPTIMIZATION LABS property is strictly prohibited.

Access Revocation:

​EVOLVE HUMAN OPTIMIZATION LABS reserves the right to revoke the Member’s access if these terms are violated.

Lab Membership Cancellation 

The member is hereby agreeing to a membership to EVOLVE HUMAN LABS. 

A request to cancel can be completed by filling out the Evolve Cancellation Request. It will take 5-10 days for cancellations to take effect.

Age Restrictions:

Minors under the age of 16 are not permitted to use the services at Evolve Human Optimization Labs without the supervision of a parent or legal guardian. 

Minors between the ages of 16-17 must have a parent or legal guardian complete the liability waiver and consent form in order to be present in the Lab without parent or legal guardian supervision. 

Parent/Legal Guardian Consent:

IN- PERSON signatures are required from all parents or legal guardians for the waiver and consent form for minors under 18 years of age. No digital signatures or verbal approvals will be accepted. This form will confirm that they understand the risks and responsibilities associated with the modalities and services provided by Evolve Human Optimization Labs. 

Membership Required for Entry for Parents or Legal Guardians of Minors:

Parents or legal guardians of minors under the age of 16 are required to purchase a membership for the minor and for themselves to have access to the Lab. The membership can either be individual or part of a family plan. 

Memberships purchased by a minor online cannot be activated in the Lab without the presence of the parent or legal guardian during the first visit. 

Minors with a membership are ineligible to bring a ‘friend for free’ unless the friend is 18+. 

Evolve Human Optimization Labs reserves the right to discontinue or cancel membership of a minor at any time. 

Supervision Requirements for Minors:

Minors who are between the ages of 13-15 must be accompanied by a parent or legal guardian at all times. They will not be allowed entry into the Lab without the parent or legal guardian present. 

Minors between the ages of 16-17 must have their parent or legal guardian complete a waiver during their first visit for the minor to enter and utilize Lab services in future visits without the parent or legal guardian present.

Parents or legal guardians are responsible for supervising their minor children during their visit.

Minors are expected to follow all Lab rules, including maintaining a respectful atmosphere for all guests.

Any disruptive behavior may result in immediate removal from the Lab.

Staff members are not responsible for supervising minors but will enforce this policy to ensure compliance and safety.

Staff members reserve the right to ask any minor or adult to leave the facility if policies are not adhered to.

Emergency Procedure Guidance for Parents or Legal Guardians of Minors:

Parents and guardians should ensure that minors are aware of emergency procedures and comfortable approaching Evolve staff members should they need assistance. 

In case of any discomfort or emergency, minors should pause their service immediately and inform the nearest staff member.  Any incidents involving minors should be reported to management or owners immediately.  

Binding 

This Lab membership contract shall be considered binding upon the Member and ​EVOLVE HUMAN OPTIMIZATION LABS, and shall be upheld and enforced in accordance with the laws of ​Texas. Any legal proceedings related to this lab membership contract shall take place in courts located in ​San Antonio, ​Texas. 

Acceptance 

The below signed parties hereby enter into this lab membership contract with one another in acknowledgement and acceptance of the terms listed above.
